This week we will include information regarding safe patient care strategies as well as information about self-monitoring strategies.  Strategies regarding safe patient care:  Many strategies will focus on the deliverance of safe care. Because medication errors happen and are the outcome is frowned upon, many hospitals and the FDA have looked into ways to help alleviate this from occurring. Some strategies may include:  Rather than focusing on changing the behavior of every healthcare worker, hospitals are now trying to understand how the system failed. Did you know..... that medication Errors do not only occur within hospital settings, but they can occur in your home as well or at a skilled nursing home facility too!   Serious harmful results of medication errors may include: death, life threatening situation, hospitalization, disability, or birth defects (Center for Drug Control Evaluation and Research).  Practicing safety is best at your organization, home, and around others!!!  What can you do, as the patient?   Be your own advocate! SPEAK UP!                                      Always carry an up to date list of medications with you! Ask your provider about education of the medication! ???? ASK QUESTIONS ????
